As a way to recognize those who have served, the Town of Middletown is participating in the Military Banner Program. This program is designed to honor any active, retired or deceased military personnel who live or lived in in the Middletown area/went to Middletown High School. The cost is: Due to the rising costs for materials, please contact the town office for current pricing .  Purchased banners will be delivered to the Town and hung on the poles in the downtown business district of Main Street during the months of May and November. 

A copy of the the service member's DD214 must be provided as proof of service.  A clear photo of the Service Member must also be provided.  Applications are available at the municipal center or by clicking on the form below.
